One of the signs of Joshua Block's poor mental health is frequent self harm in the form of biting himself, banging his head against things, and punching himself.

Biting Self

Self-injurious behavior in autism involves actions such as biting, hitting, or scratching oneself, often as a response to distress or sensory overload. In the case of Josh, an autistic individual, this behavior primarily manifests as hand biting.

Before participating in streams with Mr. Based, Josh rarely exhibited this behavior; one example was a June 2023 meltdown. However, in 2024, Josh's self-injurious actions increased alongside more frequent meltdowns.

In June, 2025, during an incident where Josh believed Santos broke his nose with a soft basketball, Josh bit Jack Doherty, leaving a red welt on his skin and causing Jack to wail in pain. Josh explained, "Sorry, I thought I was biting my own arm."

Hitting His Head Against Various Objects

There are a handful of examples of Josh dinging his head against walls, poles, and even the window of a Chicago Walgreens.

Nathan's Seafood

In November 2024, Mr. Based streamed Josh gently tapping his head against a pole outside of Nathan's Seafood in NYC. [1]

Chicago Walgreens

In May 2025, during a Chicago fry-bagging incident, Josh attempted to run head first into a Walgreens window, fortunately, he did not exert full force so that Tony could hold him back from getting into this violent altercation with said window. [2]

Broken Nose Meltdown

In June 2025, two separate incidents occurred at residences associated with internet personality Jack Doherty. In this first incident, Josh reportedly put his head through drywall following an altercation with Santos, who had thrown a soft basketball at him, leading Josh to believe his nose was horribly broken. [3]

Lost Hat Incident

The second incident in June 2025 also involved Josh damaging drywall at a Doherty residence. Josh, being a captain, had lost his second-to-last West Marine official captain's hat on a yacht--an event that sent him spiraling into a full-blown frenzy. He stormed around in a blind panic, attempting to coerce Jack to call the yacht’s owner to locate his missing property, and taking it on himself to call the police. During this incident, there was, once again, a head-shaped dent in the wall. [4]

Suicide Attempts

On July 27, 2024, an incident involving Josh occurred near a Long Island Rail Road (LIRR) station in Staten Island. According to witness accounts, the situation began after Josh was reportedly removed from Richmond Republic, a local bar, for allegedly punching another patron. Following the altercation, he became increasingly agitated, shouting about "America being backwards" before heading toward the train tracks in visible distress.

Multiple bystanders, primarily teenagers, attempted to intervene after hearing screams in a nearby Wendy’s parking lot. They described Josh as being in an extreme state of distress, stimming uncontrollably for approximately 30 minutes while resisting attempts to calm him down. Josh also called his grandfather, informing him of his suicide attempt and continuing to complain about America. When someone suggested calling emergency services, and upon realizing he was being filmed, Josh reportedly became more agitated, kicking one individual and refusing assistance, saying he couldn't afford another hospital bill.

Several witnesses attempted to physically restrain him, but he continued screaming and lashing out. Eventually, a group accompanied him onto a train to St. George to ensure his safety, though they admitted lacking experience in crisis intervention. The situation was partially recorded in videos posted by the bystanders, some of which were posted by Mr. Based.\\

Threats Against His Well-Being

Josh has frequently made alarming and bizarre statements regarding his physical well-being. In addition to making self-destructive comments, he is known for threatening surreal or extreme alterations to his own body, often involving the removal or disabling of limbs or organs.

Removal Of His Hands

In a video uploaded on November 28th, 2024, Josh is recorded saying "I won't be needing these hands anymore." while making karate-chopping motions at his wrist. [5]

Removal Of His Spinal Cord

In a video uploaded on March 26th, 2025, he states: "You know what? Maybe I should pay a doctor like $2,000 to damage my spinal cords so that I can't walk. I should pay a Nigerian doctor to disable my spinal cord. I would be happier because I would be in a nursing home, so nobody could bother me anymore." [6]
